- No. of employees working in the factory
- No. of Pressure vessels
- No of power load approved & No of load additionally needed / Horse power in use
- Demand Draft for the employees taken for the DISH
- Labour welfare Fund raised for employees.
- Details of Manager along with Mobile & Email
- Photo of Proprietor / Partners (each photo in separate)
- Aadhaar card for Proprietor / Partners along with Mobile & Email
- EPF / ESI number
- Nature of work carried out under NIC (National Identification Code)
- Turn Over taken on the proceeding year & product name.
- License Renewal in years (Upto 2032)
- LIN number. (If not known, please send EPF number)
- PAN card of the employer
- Aadhaar of the employer
- Work Order copy of the employer
- Form – III attained by employer
- Partnership Deed / Any agreement of the employer
- Number of Employees working in the contract.
- Amount to be paid for the license along with the registration charges.
